Applicants should have a BA/BS and at least 1 year of related experience, with the potential for 1-3 days in office as needed. Colocation may be required and travel to work sites is expected.

Formación

• Requires BA/BS degree and at least 1 year of experience working directly with the program population or related community-based organizations.
• Experience with health care programs in community settings is a plus.
• Strong interpersonal and communication skills for field-based coordination.

Responsabilidades

• Provide integrated whole-person Care Management under the CFSP model, coordinating across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet needs.
• Offer Trauma-Informed Care by recognizing ACEs and coordinating cross-agency care for diverse needs.
• Collaborate with the Member's County Child Welfare Worker to align health care with permanency goals.
• Work with a multidisciplinary care team to coordinate care across providers and services.
• Conduct telephonic or face-to-face assessments to identify and manage member program needs.
