Job summary

Humana is seeking a Field Care Manager, Behavioral Health in Illinois to join our Medicaid team. You will work with members face-to-face or by teleconference, coordinating integrated behavioral health care and reducing gaps in care.

Requires an IL license (LCSW/LMFT/LCPC), 2+ years post-degree experience, and ability to travel regionally. This remote role includes a $5,000 hiring bonus and a strong emphasis on team collaboration and regulatory compliance.

Qualifications

• Must reside in Illinois
• Active Illinois license in LCSW, LMFT or LCPC (No supervisees or provisional licenses)
• 2+ years of post-degree clinical experience in behavioral health setting
• Case management experience with complex SMI/SUD/SED
• Ability to travel to region-based facilities and homes for face-to-face assessments
• Experience with electronic medical records and MS Office
• Valid driver's license, car insurance, and reliable transportation

Responsibilities

• Utilize evidence-based behavioral health services through care coordination, crisis intervention, peer support, and collaboration with providers.
• Provide integrated support to members with behavioral health conditions including children, adolescents, adults with SMI and SED, SUD and justice-involved members.
• Engage members in their communities, meeting face-to-face to build trust and coordinate care.
• Coordinate behavioral health and medical services with provider engagement and treatment plans.
• Improve health literacy while addressing social needs to impact outcomes and well-being.
• Lead the member's interdisciplinary care team (ICT) and oversee care planning and transitions.
• Facilitate ICT meetings to ensure communication among providers and care teams.
• Support transition of care efforts proactively.
• Collaborate with internal departments, providers, and community resources for seamless, culturally competent care.
