Job summary

STGi seeks a Case Manager to support HUD/VASH veterans in Los Angeles. You will provide clinical case management to a caseload of up to 25 veterans, coordinating housing stability plans and connecting clients to housing resources and VA services.

In this role, you will conduct comprehensive assessments, educate veterans on life skills, and collaborate with VA and community partners to ensure wrap-around care and housing stability.

Responsibilities

• Provide direct clinical case management to a caseload of up to 25 Veterans experiencing homelessness.
• Assist Veterans in navigating housing authorities and securing permanent supportive housing with Housing Locator support.
• Conduct comprehensive screenings to collect functional, financial, housing, and health information for case planning.
• Provide education on community living skills including money management, shopping, safety, and basic hygiene/nutrition.
• Refer and link to VA medical facilities, regional offices, and community services for health care, benefits, education, and other needs.
• Provide crisis management and periodic 24-hour coverage as needed.
• Develop collaborative relationships with service providers to ensure continuity of care and wrap-around services.
• Advocate for Veterans to access needed services.
• Attend daily/weekly meetings, supervision, and case conferences.
