Job summary

The Center for Human Development (CHD) is seeking an Outreach Where you Live (OWLS) Unsheltered Outreach Caseworker to join our new Unsheltered Outreach program serving the Litchfield county area in Torrington, CT.

You will oversee a variety of cases, maintain a daily street outreach schedule, coordinate with external support providers, and follow CHD and Western CT Mental Health Network protocols to ensure open communication and accountability among collaborating agencies.

Qualifications

• High school diploma or GED required.
• Two years’ experience working with adults with severe and prolonged mental illness, addiction/recovery or unsheltered populations.
• Lived experience encouraged to apply.
• Bachelor’s degree in human services preferred.

Responsibilities

• Oversee a variety of cases through different venues in the Litchfield county area.
• Maintain a daily street outreach schedule and coordinate with external support providers.
• Work within program models and protocols established by partner networks to ensure open communication and accountability.
