The Center for Human Development (CHD) seeks an OWLs Unsheltered Outreach Caseworker in Waterbury, CT to join our new Unsheltered Outreach program. You will provide street outreach and case management to adults in need, coordinating with Western CT Mental Health Network and Coordinated Access Network.

This full-time, on-site role requires driving, a valid vehicle, CORI check, and the ability to work flexible hours.

Qualificações

• Must have high school diploma or GED.
• Two years’ experience with adults with mental illness, addiction/recovery or unsheltered populations.
• Lived experience encouraged to apply.
• Bachelor’s degree in human services preferred.
• Reliable, registered & insured vehicle with driving record check.
• CORI background check required.

Responsabilidades

• Oversee a variety of cases through different venues in Waterbury, CT.
• Provide tools to unsheltered individuals to help end homelessness.
• Maintain a schedule of street outreach and external support providers.
• Work within program models and established protocols with partner networks.
