Job summary

Center for Human Development, Inc. (CHD) seeks an OWLS Unsheltered Outreach Caseworker in Waterbury, CT. The role involves field outreach and coordinating with partner agencies to help individuals end homelessness.

You will manage a daily outreach schedule, drive as needed, and work within established protocols with the Western CT Mental Health Network and Coordinated Access Network. A high school diploma is required, with a preference for a bachelor’s degree in human services.

Qualifications

• High school diploma or GED required.
• Two years’ experience with adults with severe mental illness, addiction or unsheltered populations.
• Ability to drive, own vehicle, and valid driving record.
• Field-based role; not an office position and requires driving in Litchfield county.

Responsibilities

• Oversee a variety of cases across different venues in Waterbury, CT.
• Maintain a schedule including hours of street outreach and coordination with external providers.
• Follow CHD and Western CT Mental Health Network protocols for outreach.
