Job summary

Care Lync in Greenville, SC is seeking a dedicated Support Coordinator / Service Coordinator to develop and manage individualized care plans for adults with IDD and disabilities. You will broker services, monitor outcomes, and coordinate housing, medical, social, and community supports to help clients live independently.

Ideal candidates have a BS/BA and at least 3 years of related case management experience, plus proficiency with Microsoft Office and state databases.

Qualifications

• Minimum of BS/BA degree in related field.
• At least 3 years of case management experience with intellectual/developmental or physical disabilities.
• Proficiency with Microsoft Office and state-maintained databases.
• CPR/First Aid certification and state clearances must be current.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop Person-Centered Service Plans and update annually or with major changes.
• Provide information to help participants choose providers confidently.
• Ensure ISP aligns with Behavioral and Crisis Plans when applicable.
• Report incidents of abuse, neglect, or exploitation immediately.
• Coordinate with care teams to ensure seamless physical, behavioral, and support services.
• Travel to participants as needed (up to 50%).
