Job summary

Elevance Health is seeking a LTSS Service Coordinator - RN Clinician (Case Manager) to manage members' long-term services and supports across health and behavioral care. This field-based role travels to client sites, builds care plans, and coordinates with the health care team to meet each member's needs.

The role requires RN licensure in Ohio, a high school diploma, and 3+ years in case management or related work.

Qualifications

• Performs telephonic and face-to-face assessments to identify needs and coordinate care.
• Manages members with chronic illnesses to optimize health benefits.
• Collaborates with medical directors and interdisciplinary teams for care plans.
• Develops individualized care plans with member and family.
• Ensures access to services within the benefits structure.

Responsibilities

• Perform telephonic and face-to-face assessments to identify needs and coordinate care.
• Coordinate care with health team and members.
• Develop, monitor, and revise member care plans.
• Assist with authorizations/referrals per benefits.
• Interface with Medical Directors and care teams to solve issues.
