Job summary

ForzaCare in Virginia is seeking a Field Medical Case Manager to coordinate care for injured workers, liaise with providers, employers, and insurers, and ensure timely, transparent case management. You will travel to appointments, develop recovery goals, monitor progress, and maintain strong client relationships while adhering to safety and regulatory standards.

Active RN license is required; CCM/CIRS or other credentials are preferred; Maryland WCCM is a plus.

Qualifications

• RN licensure is required.
• CCM, CIRS or other case management credentials preferred.
• Maryland WCCM preferred.
• Must meet state licensure and certification requirements.
• Workers' compensation case management experience preferred.
• Valid driver’s license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits to workplaces as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
