Job summary

HireTalent is seeking a Field Care Manager Nurse II (RN Case Manager) to support Medicare and Medicaid members through comprehensive care coordination, clinical assessment, and ongoing case management. This field-based role involves in-home and community-based assessments and development of personalized care plans.

The position emphasizes independent work, strong communication, and meaningful impact on member health outcomes through collaboration with caregivers and providers within Michigan.

Qualifications

• Active Michigan RN license with no disciplinary actions.
• Must reside in Michigan and able to travel within Wayne/Macomb Counties.
• Minimum of 2 years of healthcare and/or case management experience.
• Familiarity with EMR, Word, Excel, and Outlook; able to work remotely as needed.

Responsibilities

• Manage a caseload of Medicare and Medicaid members and coordinate in-home assessments.
• Provide clinical guidance and conduct medical complexity assessments.
• Coordinate care with interdisciplinary care team and community resources.
• Assist with social determinants of health needs and housing/community services.
