Job summary

Elevance Health in Seven Hills, OH, is seeking an LTSS Service Coordinator – RN Clinician (RN Case Manager) to manage member cases and develop care plans.

Qualifications

• Active RN license in Ohio required.
• Minimum 3 years of case management experience.
• High school diploma or GED required; BA/BS preferred.

Responsibilities

• Perform telephonic and face-to-face functional assessments for members.
• Identify members for high-risk complications and coordinate care.
• Manage members with chronic illnesses, co-morbidities, and/or disabilities.
• Obtain thorough and accurate member history to develop individual care plans.
• Establish short and long-term goals in collaboration with members and their support systems.
• Develop care plans for services and ensure member access to those services.
• Facilitate authorizations/referrals for utilization of services as appropriate.
• Interface with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams on care management.
• Assist in problem-solving with providers, claims, or service issues.

About This Role

The LTSS Service Coordinator – RN Clinician (RN Case Manager) is responsible for the overall management of member cases, developing and monitoring care plans to meet individual needs. This role is ideal for experienced RNs looking to make a significant impact in a field-based position across Ohio.

Highlights

• Pay: $36.38/hr to $45.48/hr
• Location: Seven Hills, OH
