Job summary

Constellation Home Care in the Fairfield/Bridgeport area is seeking a compassionate Registered Nurse (RN) Case Manager to join our field-based team. You will drive to patients' homes, assisted living facilities, and skilled nursing facilities to deliver skilled nursing care.

You will assess patients, manage medications and wound care, develop and coordinate Plans of Care with the care team, and educate families. Strong Oasis and iPad documentation experience preferred.

Qualifications

• RN licensed in the state of employment.
• Graduate of an accredited RN program.
• At least 1–2 years of RN experience; home care preferred.
• OASIS experience strongly desired.
• Experience managing catheters, wound care, ostomy care is a plus.

Responsibilities

• Provide skilled nursing and head-to-toe assessments for adults/geriatric patients.
• Oversee medication administration and wound care as prescribed.
• Provide home health visits to an assigned caseload (admissions, recertifications, discharges).
• Develop the Plan of Care with involved health care professionals.
• Be knowledgeable in current OASIS assessment for admissions, transfers, discharges.
• Educate patients and caregivers on disease, medications, plan of care.
• Report to Clinical Manager daily and document using provided iPad within required time frames.
