UniversaCare Inc. seeks experienced Registered Nurses to join our home care team as Home Care Case Managers in Connecticut. This role centers on coordinating care, performing patient assessments, and guiding families to improve outcomes.

You will collaborate with physicians and caregivers, maintain documentation, and support patients across New Haven and Hartford Counties. Flexible full-time or part-time schedules are available, with competitive compensation and a sign-on bonus.

Qualifications

• Active Connecticut RN License.
• Home Care or Hospital Case Management experience preferred.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and manage multiple cases.
