Your Health is seeking a Registered Nurse to serve patients across the Columbia area as a Visiting Nurse and Nurse Case Manager. The role requires visiting homes and long-term care facilities, coordinating with providers, initiating telemedicine visits, and ensuring care plans meet regulatory standards.

The ideal candidate is licensed as an RN, with at least three years of clinical experience, strong communication skills, and the ability to travel daily.

Qualifications

• Must be licensed as a registered nurse in good standing.
• Three (3) years clinical experience preferred.
• Experience in community settings preferred.

Responsibilities

• Visit patients in homes and facilities to assess care needs.
• Coordinate with providers and initiate telemedicine visits.
• Document care management activities and maintain care plans.
