Job summary

Your Health seeks a Registered Nurse to join our growing primary care family as a Visiting Nurse/Nurse Case Manager. You will visit patients at homes and facilities, initiate telemedicine, and coordinate with providers to ensure quality, compliant care. A valid nursing license and ability to travel daily are required.

Qualifications

• Must be a licensed nurse in good standing.
• Minimum three years of clinical experience preferred.
• Experience in community settings preferred.
• Strong written and verbal communication skills.
• Ability to manage multiple responsibilities with sound judgment.

Responsibilities

• Facilitate receiving medical records from primary providers and specialists.
• Review medical records and complete patient consents.
• Enroll eligible patients in Care Management and initiate Plans of Care.
• Capture diagnoses with high specificity and address gaps.
• Complete assessments and screenings (AWV, cognitive, SDoH, ACP).
• Evaluate home health, hospice, and RPM opportunities as appropriate.
• Coordinate care and reconcile medications and therapies.
• Document Care Management activities and maintain care plans.
