Job summary

Your Health is seeking a Registered Nurse to serve our patients across the Columbia area. The Nurse Case Manager will travel to patients’ homes and facilities, coordinating with providers to initiate telemedicine visits and ensure quality care.

The role requires visiting patients in various settings, enrolling eligible patients in Care Management, and logging comprehensive care plans with strong collaboration across the care team. A valid RN license and reliable transportation are essential.

Qualifications

• Must be a Registered nurse in good standing.
• Minimum 3 years clinical experience preferred.
• Experience in community settings preferred.
• Strong written and verbal communication skills.
• Ability to travel to patients' homes and facilities.
• Basic computer knowledge.

Responsibilities

• Facilitate receiving medical records from the patient’s primary provider and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management and initiate a Care Plan of Care.
• Coordinate with care team and telemedicine visits as needed.
• Reconcile prescribed and OTC medications; provide education.
• Evaluate for home health, hospice, or RPM devices when appropriate.
• Document Care Management activities accurately.
