Job summary

Your Health Organization in the Columbia area is seeking a Registered Nurse to serve as a Nurse Case Manager. The role requires visiting patients in homes and facilities, coordinating with providers, and initiating telemedicine visits to ensure comprehensive care.

Travel is required daily, with a focus on improving patient outcomes across service areas like Columbia, Irmo, Blythewood, and Lexington. Strong communication and collaboration with care teams are essential.

Qualifications

• Must be a Registered nurse. License must be in good standing with appropriate board/issuer.
• A minimum of three (3) years clinical experience preferred.
• Experience in community settings preferred.

Responsibilities

• Visit patients in homes and facilities to evaluate care needs and coordinate with providers.
• Enroll patients in Care Management and initiate a Care Management Plan of Care when eligible.
• Communicate and coordinate care with the patient, families, and interdisciplinary teams; include telemedicine when appropriate.
• Ensure documentation, track care activities, and support RPM or other tools as applicable.
