Job summary

CVS Health is hiring a full-time RN to develop and support health service strategies across New Jersey with up to 75% travel. You will perform home visits, assessments, and care planning, coordinating with providers and care teams to promote high-quality member care.

Requirements include NJ RN licensure, 3+ years clinical experience, and NJ Choice Certification. Residency in Essex or nearby counties is preferred.

Qualifications

• Active, unrestricted RN license in New Jersey.
• Minimum of 3 years of clinical experience in hospitals, home health, or ambulatory care.
• Residency in Essex or adjacent counties.
• Willingness to travel up to 75% within Ocean and surrounding counties.
• NJ Choice Certification requirements: exam score 80%+, state training modules, field mentoring.

Responsibilities

• Conduct face-to-face home visits for MLTSS and/or D-SNP/FIDE members.
• Perform in-person assessments to determine medical needs and referrals.
• Coordinate and collaborate with providers and interdisciplinary care teams.
• Schedule and participate in interdisciplinary meetings to advocate for members.
• Document assessments and care plans in the electronic health record.
• Mentor new staff after achieving proficiency.
