Oak Street Health in Simpson, LA is seeking an experienced RN, Case Manager to lead care coordination for high-risk patients in a field-based role. You will partner with PCPs and interdisciplinary teams to prevent admissions, manage transitions of care, and drive efficient resource use while maintaining focus on patient-centered outcomes; travel may be required within communities served.

Qualifications include Spanish fluency, a current RN license, CCM certification, 6–8 years of nursing

Qualificações

• Fluency in Spanish or other languages spoken by the communities served.
• Current RN license in the assigned state; Bachelor’s degree in nursing preferred.
• Minimum 6–8 years of nursing experience.
• Certified Case Manager (CCM) required or pursued within 12 months of hire unless applicable experience.
• 2+ years in transitional nursing, discharge planning, or home health preferred.

Responsabilidades

• Manages an assigned caseload of complex patients in a value-based care environment to reduce admissions and readmissions.
• Takes ownership of care coordination and serves as primary point of contact across the care team.
• Oversees transitions of care, including assessments, medication reconciliation, and collaboration with PCPs.
• Collaborates with PCP, family, Social Worker, Behavioral Health Specialists, and other team members to align care goals.
• Documents key clinical assessments post-admission and supports in-center care planning.
• Facilitates in-person interdisciplinary care planning meetings.
• Performs nursing assessments and provides patient education on chronic conditions and transitions of care.
• Educates patients and families to empower self-management and advocacy.
