Job summary

Humana/iCare in Milwaukee, WI seeks an RN Care Manager to provide case management for adults in the Family Care Partnership program, including home and community visits in Milwaukee and surrounding counties. You will report to the Manager of Care Management Support and ensure members receive appropriate care and access to services.

Requirements include Wisconsin RN license in good standing and at least 2 years of health care experience; BSN preferred.

Qualifications

• Must be a Registered Nurse licensed in Wisconsin in good standing.
• Two years of related health care experience.
• BSN preferred.
• Previous home health, managed care or case management experience.
• General knowledge of Medicaid and Medicare benefits.

Responsibilities

• Complete the nursing portion of the comprehensive assessment and provide input to the member's care.
• Monitor wellness and health promotion activities based on the member's assessment and outcomes.
• Re-evaluate health and wellness at appropriate intervals to manage ongoing issues.
• Perform routine health monitoring activities based on the member's needs.
• Build constructive working relationships with members and families.
• In crisis situations, assess psychiatric/medical problems and coordinate interventions.
• Evaluate ability to safely manage medications and treatments.
• Provide or arrange services that meet the member's needs and monitor effectiveness.
• Assist with input for the QI/QA program.
