Job summary

FreedomCare LLC is hiring a Care Manager to join our Massachusetts team and conduct initial home assessments to determine eligibility for services. This field-based role travels to patient homes in the greater Boston area to assess needs and coordinate care with the care team.

You will coach caregivers, complete monthly visits, and document interactions in our systems. A bachelor’s degree and MA social worker license or equivalent experience are required, plus 2+ years in health coaching or care

Qualifications

• Bachelor’s degree and MA social worker license (or equivalent).
• 2+ years health coaching, care coordination or related experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home assessments for caregiver quality and patient needs.
• Provide coaching and education to caregivers via in-person demonstrations and LMS trainings.
• Support patients with service questions, goals and next steps within non-clinical scope.
• Document coaching interactions, outcomes and follow-up tasks in required systems.
• Coordinate care with Care Team, Clinical, Physicians and leadership for escalation as needed.
