UnitedHealth Group is seeking a Field-Based Registered Nurse in the Dallas, TX area to assess, plan, and implement individualized care strategies. You will identify social service referrals and manage care plans as a single point of contact for coordinated care.

Travel up to 75% within the Dallas region; telecommuting options apply per policy. A Texas RN license and 2+ years in community healthcare are required; proficiency with Microsoft Office is expected.

Qualifikationen

• Current unrestricted Texas RN license.
• 2+ years of experience in a community health or healthcare setting.
• Intermediate level of proficiency with Microsoft Office applications.
• Valid driver''s license and reliable transportation; ability to travel up to 75% in the assigned region to visit Medicaid members in homes or other settings.
• Reside in or within commutable driving distance to Dallas, TX.

Aufgaben

• Assess, plan and implement care strategies individualized for patients and directed toward appropriate, least restrictive level of care.
• Identify and initiate referrals for social service programs (financial, psychosocial, community and state support).
• Manage the care plan throughout the continuum of care as a single point of contact; communicate required health information to stakeholders to ensure coordinated care.
• Advocate for patients and families to ensure needs and choices are represented and supported by the healthcare team.
• Travel up to 75% within the Dallas, TX area and surrounding areas.
