Job summary

UnitedHealthcare is seeking a Field Based HSS Clinical Coordinator RN to support Dallas-area patients with individualized care plans and timely referrals. This role requires travel up to 75% within the Dallas area and commuting to patient homes or facilities.

You will assess and implement care strategies, coordinate with the health care team and social services, and ensure high-quality, equitable care for Medicaid/Medicare populations in the region.

Qualifications

• 2+ years of experience in a community health or healthcare setting.
• Intermediate level of proficiency with Microsoft Office applications.
• Valid driver’s license and reliable transportation; ability to travel up to 75% in the Dallas, TX region.
• Reside in or within commuting distance to Dallas, TX.

Responsibilities

• Assess, plan and implement care strategies individualized by patients.
• Identify and initiate referrals for social services programs.
• Manage care plans as a single point of contact; communicate health information to stakeholders
• Advocate for patients and families to ensure needs are met by the health care team.
• Travel approximately 75% within the Dallas area and surrounding regions.
