Job summary

UnitedHealthcare is hiring a Field Based Registered Nurse to deliver patient-centered care in the Dallas, TX region. This home-based role requires you to travel up to 75% within the assigned area and coordinate care across settings for Medicaid members.

You must hold an unrestricted Texas RN license and have at least 2 years of relevant healthcare experience. A valid driver’s license and reliable transportation are essential, with telecommuting flexibility within the Dallas area.

Qualifications

Current unrestricted Texas RN license.
2+ years of experience in a community health or healthcare setting.
Valid driver's license, access to reliable transportation, ability to travel up to 75% in assigned region to visit Medicaid members in their homes and/or other settings.

Responsibilities

Assess, plan and implement care strategies individualized by patients and directed toward the most appropriate level of care.
Identify and initiate referrals for social service programs.
Manage the care plan throughout the continuum of care as a single point of contact; communicate required health information to ensure quality, coordinated care.
Advocate for patients and families as needed to ensure needs and choices are represented and supported.
Travel 75% of the time within the Dallas, TX area and surrounding areas.
