Job summary

UnitedHealthcare is seeking an RN Assessor to enroll members into the NYMAP plan and develop person-centered service plans through home visits in the Brooklyn and Richmond area. You will collaborate with NYMAP care managers to assess needs and implement services that support member goals.

The role is primarily field-based with substantial in-home visits, and offers telecommute flexibility where feasible. A NY RN license and strong community health experience are required.

Qualifications

• Associates degree and NY RN license required.
• 4+ years in community health healthcare role.
• 2+ years in long-term care, home health, hospice or similar.
• 1+ years of community case management experience coordinating care for individuals with complex needs.
• Must have NY state ID and valid US driver\'s license.

Responsibilities

• Assess, plan and implement individualized care strategies for members.
• Perform NYS UAS assessment in the members\' homes annually.
• Identify referrals for social services including financial, psychosocial, community and state supportive services.
• Develop a Person-Centered Service Plan using the UAS-NY.
• Manage the service plan across the continuum of care with the care management team.
• Communicate health-related information to stakeholders to ensure coordinated care.
• Advocate for members and families to support their wants and needs.
