Job summary

UnitedHealth Group seeks an RN Assessor to enroll members in the NYMAP plan and develop person-centered service plans in the member’s home setting. This field-based role requires extensive travel within Queens, Bronx, and Manhattan.

You will join a collaborative care management team, performing annual UAS assessments, coordinating with care managers, and advocating for member needs while delivering high-quality, equitable care.

Qualifications

• RN license (New York) required.
• 4+ years of experience in community health settings.
• 1+ years in case management or coordinating care for complex needs.
• Ability to travel within the assigned region to visit Medicaid members in homes or other settings.

Responsibilities

• Assess, plan and implement care strategies individualized per member.
• Perform NYS UAS Assessment in the member's home annually and as needed.
• Develop a Person Centered Service Plan using UAS-NY.
• Maintain and monitor the service plan across the continuum of care.
• Communicate health information to all stakeholders for coordinated care.
• Advocate for members and families to ensure needs are met by the care team.
