Job summary

UnitedHealthcare seeks an RN Assessor to enroll members in the NYMAP plan and develop person-centered service plans in Western New York. The role is field-based, requiring 75–100% in-home or in-community visits, with telecommuting flexibility.

Candidates must hold an NY RN license and have 4+ years in community health, plus 1+ year in managed care settings. Travel across Western NY is expected.

Qualifications

• Associate's degree required.
• Current, unrestricted RN license in New York state.
• 4+ years of experience in community health settings.
• 2+ years in long-term care, home health, hospice, public health or assisted living.
• 1+ years of community case management experience.

Responsibilities

• Assess, plan and implement care strategies individualized by member and directed toward the least restrictive level of care.
• Perform the NYS UAS Assessment in the member's home at least annually and as needed.
• Identify and initiate referrals for social service programs including financial, psychosocial, community and state supportive services.
• Utilize the UAS-NY to develop a Person Centered Service Plan that meets the member's needs.
• Manage the person centered service plan throughout the continuum of care as part of the care management team.
• Communicate health-related information to stakeholders to ensure quality, coordinated care and services.
