Job summary

UnitedHealthcare, part of UnitedHealth Group, seeks an RN Assessor to enroll NYMAP members and develop person-centered service plans. This field-based role requires 75–100% time in the field visiting members in Queens, Bronx, and Manhattan.

You will perform NYS UAS assessments, coordinate with care managers, and advocate for members’ needs. Telecommute options may apply in designated areas.

Qualifications

• Associate's degree and NY RN license required.
• 4+ years in community health care setting.
• 2+ years in long-term care, home health, hospice, public health or assisted living.
• 1+ years of community case management experience coordinating care for complex needs.
• Willingness to travel within assigned region to visit Medicaid members.
• Must have active New York state ID.

Responsibilities

• Assess, plan and implement member-centered care strategies.
• Perform NYS UAS Assessment in the member's home annually.
• Develop a Person Centered Service Plan using UAS-NY.
• Manage the service plan across the continuum of care with the care team.
• Communicate health information to stakeholders for coordinated care.
• Advocate for members and families to ensure needs and choices are represented.
