Job summary

UnitedHealth Group is seeking an RN Assessor to enroll members into the NYMAP plan and collaborate with enrollees to shape their person-centered service plan. This field-based role engages directly with members in their communities.

Based in Western New York, the RN Assessor will conduct home assessments, support referrals, and advocate for members and families while following NYS UAS guidelines.

Qualifications

• Associate's degree required.
• 4+ years in community health setting.
• 2+ years in long-term care, home health, hospice, public health, or assisted living.
• 1+ years in community case management for individuals with complex needs.
• Current, unrestricted RN license in New York.
• Must hold active New York state ID.

Responsibilities

• Assess, plan and implement individualized care strategies for members.
• Perform the NYS UAS Assessment in the member's home at least annually.
• Identify and initiate referrals for social service programs.
• Utilize the UAS-NY to develop a Person Centered Service Plan.
• Manage the person-centered service plan throughout the continuum of careCommunicate required health-related information to stakeholders.
• Advocate for members and their families to ensure their needs are represented.

• Work Style: Field-based
• Schedule: Full Time, Day shift
