Job summary

Qualifications

• RN license issued by New York State is required.
• Multiple years of community health experience required.
• Experience in long-term care, home health, hospice or related fields preferred.
• Experience coordinating care for individuals with complex needs.
• Ability to travel within assigned Western NY regions to member homes.
• Must hold an active New York state ID.

Responsibilities

• Assess and plan individualized care strategies for members.
• Perform NYS UAS assessments in member homes annually or as needed.
• Develop Person Centered Service Plans with stakeholders.
• Manage and adjust care plans across care continuum.
• Communicate health information to ensure coordinated, timely care.
• Advocate for members and families within the care team.
