Job summary

UnitedHealth Group is seeking an RN Assessor to support the NYMAP plan in Brooklyn, NY and surrounding areas. This field-based role requires 75-100% travel to members' homes and community settings.

Primary responsibilities include assessing care strategies, performing UAS assessments, and developing person-centered care plans. Telecommuting is available, with a comprehensive benefits package and competitive hourly pay.

Qualifications

• Associate’s degree is required.
• Current, unrestricted RN license for New York state.
• 4+ years of experience in community health.
• 2+ years of experience in long-term care, home health, hospice, public health or assisted living.
• 1+ years of community case management experience coordinating care for individuals with complex needs.
• Ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
• Must hold active New York state ID.
• Access to reliable transportation and valid US driver's license.

Responsibilities

• Assess, plan and implement care strategies individualized by members toward the most appropriate, least restrictive level of care.
• Perform the NYS UAS Assessment in the member's home at least annually and as needed; identify and initiate referrals for social service programs.
• Utilize the UAS-NY to develop a Person-Centered Service Plan that meets the member's needs.
• Manage the person-centered service plan throughout the continuum of care as part of the care management team.
• Communicate health-related information to ensure quality coordinated care and services.
• Advocate for members and their families to ensure needs and choices are represented and supported.
