Job summary

Primecare Home Care Services, LLC, powered by Flournoy Health Systems, is seeking a Registered Nurse (RN) to provide clinical oversight and coordination of home-based care within an assigned Georgia service area. The RN will perform assessments, develop plans of care, supervise staff, and support physicians and families to ensure compliant, patient-centered care.

Qualifications

• Current, active RN license to practice in Georgia.
• Basic Life Support (BLS) certification.
• Experience with home health/home care and Medicaid-funded services is preferred.

Responsibilities

• Perform initial assessments and reassessments; develop Plans of Care.
• Provide clinical supervision and education to LPNs, caregivers, and families.
• Coordinate supervisory and IDT visit schedules with the operations team.
• Conduct supervisory visits for skilled clients and unannounced visits when needed.
• Review and sign off on clinical documentation; manage POTs and orders.
• Monitor patient conditions, identify changes, and coordinate interventions.
• Educate clients and caregivers on diagnoses, medications, safety, and care needs.
• Ensure HIPAA and agency requirements are met; provide in-field nursing support as needed.
