Qualifications

• Current, active, and unencumbered RN license to practice in Georgia.
• Current BLS certification.
• Ability to assess and manage medically complex clients.
• Ability to develop and maintain individualized care plans.

Responsibilities

• Perform initial assessments and reassessments; develop Plans of Care.
• Provide clinical supervision to LPNs and caregivers.
• Coordinate supervisory visits and IDT schedules; ensure timely visits.
• Review and sign clinical documentation; ensure alignment with Plan of Care.
• Manage physician POTs, orders, and clinical documentation.
• Monitor client conditions; coordinate necessary clinical interventions.
