Job summary

Primecare Home Care Services in Georgia is seeking a Registered Nurse to provide clinical oversight and coordination for clients receiving home-based services in their community. The RN will perform assessments, develop individualized care plans, and ensure compliance with regulatory requirements and documentation standards.

The role covers in-field coverage, supervision of LPNs and caregivers, collaboration with physicians and families, and ongoing education on diagnoses and treatments.

Qualifications

• Current, active Georgia RN license.
• BLS certification.
• Ability to develop and maintain individualized Plans of Care.
• Experience in home health or community-based nursing preferred.
• Strong clinical supervision and education skills.

Responsibilities

• Complete initial assessments and reassessments; develop Plans of Care.
• Provide clinical supervision to LPNs and caregivers.
• Coordinate supervisory and IDT visit schedules; ensure visits are completed.
• Conduct supervisory visits for skilled clients and follow-ups.
• Review and sign off on clinical documentation.
• Manage physician POT and coordinate orders.
• Monitor complex conditions and coordinate interventions.
• Educate clients and families on diagnoses, meds, safety.
• Ensure HIPAA compliance and regulatory requirements.
