Job summary

Primecare Home Care Services Inc. is seeking a Registered Nurse (RN) to provide clinical oversight and coordination of home-based care in Georgia. The RN will complete assessments, develop plans of care, supervise LPNs and caregivers, and ensure regulatory compliance across Medicaid-funded services.

Responsibilities include reviewing POTs, documenting care, collaborating with physicians, families, and team members, and delivering in-field nursing support to maintain continuity of care.

Qualifications

• Current RN license to practice in the State of Georgia.
• Basic Life Support (BLS) certification.
• Experience with Medicaid-funded home and community-based services preferred.

Responsibilities

• Complete initial assessments and reassessments and maintain Plans of Care.
• Provide clinical supervision to LPNs and caregivers.
• Coordinate supervisory and IDT visit schedules and ensure visits are completed timely.
• Review and sign off on clinical and care-delivery documentation.
• Monitor and manage physician Plans of Treatment and clinical orders.
• Communicate with physicians, case managers, families, and caregivers to support continuity of care.
• Provide client and caregiver education regarding diagnoses, medications, safety, and care needs.
• Ensure compliance with Medicaid, HIPAA, and agency requirements.
