Job summary

Primecare Home Care Services, Inc. in Georgia seeks an RN Supervisor to oversee admissions, assessments, care plans, and clinical documentation for home health clients.

You will provide clinical leadership to nursing staff, monitor service delivery, and ensure compliance with Medicaid and state requirements. The role requires active Georgia RN licensure, 2+ years of nursing experience (home health preferred), and willingness to travel within the coverage area for supervisory visits.

Qualifications

• Active RN license in Georgia.
• Minimum 2 years nursing experience; home healthcare experience preferred.
• Prior supervisory or clinical leadership experience preferred.
• Knowledge of Medicaid regulations, state requirements, and home healthcare program rules.
• Strong clinical assessment, organizational, communication, and problem-solving skills.
• Ability to work independently while managing multiple priorities and a defined geographic coverage area.
• Willingness to travel throughout the assigned coverage area for supervisory visits.

Responsibilities

• Oversee admissions, assessments, reassessments, care plans, supervisory visits, and clinical documentation for clients within the assigned coverage area.
• Monitor caregiver shifts and service delivery to ensure services are provided as authorized and per the plan of care.
• Provide clinical guidance to nursing staff and address changes in client condition or other clinical concerns.
• Complete required supervisory visits, assessments, or other clinical duties when assigned staff are unavailable.
• Provide clinical direction, support, and accountability to nursing staff within the coverage area.
• Monitor staff performance and address concerns related to client care, documentation, and visits.
• Assist with recruitment, onboarding, training, and evaluation of nursing personnel as needed.
• Identify staffing and clinical coverage gaps and collaborate with leadership to address them.
• Maintain accountability for clinical compliance, including visits, assessments, documentation, and program requirements.
• Identify and address compliance gaps, clinical risks, and documentation concerns promptly.
• Participate in audits, corrective action plans, and quality improvement initiatives.
• Ensure incidents and reportable concerns are documented, reported, and resolved.
• Serve as a clinical point of contact for clients, caregivers, families, physicians, case managers, and other healthcare professionals.
• Provide education and support regarding client care, care plans, and program requirements.
• Collaborate with interdisciplinary team members to promote safe and appropriate client care.
• Respond to clinical concerns and emergencies and elevate issues as needed.
