Job summary

Adaptive Home Health is seeking a Registered Nurse (RN) Case Manager to deliver high-acuity home health care across Texas. You will manage assessments, document visits including OASIS, and coordinate with physicians and the care team while advocating for patients and families.

You will lead wound care and infusion therapies, ensure care continuity, and mentor LVNs and HHAs in a fast-paced, autonomous field role with strategic impact.

Qualifications

• Active Texas RN license.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) required.
• Texas driver’s license; field-based work.
• Strong time management and organization.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care focusing on wound care and infusion therapy.
• Communicate plan of care with physicians, care team, patients and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Coordinate care and follow-ups to keep patients progressing toward goals.
• Supervise LVNs and HHAs as applicable.
• Participate in care conferences to align on quality and safety.
