Job summary

Adaptive Home Health is seeking a Registered Nurse (RN) - Case Manager for field-based, weekend coverage in Texas. You will own patient assessments and skilled visit documentation including OASIS, deliver high-acuity home health care focusing on wound care and infusions, and coordinate with physicians and families.

You will advocate for patients, drive care plans, supervise LVNs and HHAs when applicable, and help drive outcomes with a flexible schedule and supportive team in a mission-driven

Qualifications

• Current Texas RN license.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR required (BLS).
• Previous home health experience.
• Current Texas driver's license and field-based work.
• Strong time management and organization in a high-autonomy environment.
• Clear, professional communication with patients, families, and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care, with focus on wound care and infusion therapy.
• Build and maintain a clear plan of care through consistent communication with physicians, care team, patients, and families.
• Serve as the patient advocate and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination, timely follow-ups, and barrier resolution.
• Coordinate services (PT/OT/ST/MSW/HHA) to ensure plan of care is executed.
• Administer medications and treatments as ordered, and provide patient and caregiver teaching.
• Provide supervision and guidance to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality, safety, and clinical outcomes.
