Adaptive Home Health is seeking a Registered Nurse (RN) – Case Manager to join our field-based team. You will own patient assessments and provide high-acuity home health care with a focus on wound care and infusion therapy, while ensuring a clear plan of care through collaboration with physicians and families.

Ideal candidates have 3+ years of nursing experience and Texas RN licensure, with strong time management and communication skills.

Qualifications

• Must hold a Texas RN license and have 3+ years of clinical nursing experience.
• Experience with wound care and infusion therapy is required.
• CPR (BLS) certification and a valid Texas driver's license are essential.
• Strong communication with patients, families and providers is expected.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care focusing on wound care and infusions.
• Build and maintain a clear plan of care through consistent communication with physicians and care teams.
• Coordinate services to ensure continuity of care across disciplines and advocate for patients.
• Drive case management responsibilities including follow-ups and barrier resolution.
• Coordinate with PT/OT/ST/MSW/HHA and supervise LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality and outcomes.
