Job summary

Adaptive Home Health is seeking an experienced Registered Nurse (RN) Case Manager to deliver high-acuity home health care in patients' homes across Texas. The role emphasizes autonomy, clinical judgment, and advocacy for patients and families.

You will document care, coordinate with physicians and care teams, and ensure a clear plan of care while leveraging our charting and scheduling tools to reduce documentation burden. The schedule is full-time, Monday–Friday with some weekends as needed.

Qualifications

• Current Texas RN license.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) required.
• Current Texas driver’s license and field-based work.
• Strong time management and organization in a high-autonomy environment.
• Clear, professional communication with patients, families, and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care, focusing on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians, care team members, patients, and families.
• Serve as patient advocate and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination, follow-ups, and barrier resolution.
• Coordinate services (PT/OT/ST/MSW/HHA) to ensure the plan of care is executed.
• Administer medications and treatments as ordered, and provide patient and caregiver teaching.
• Coordinate with internal clinical and operational support teams for reliable, safe care.
• Provide supervision and guidance to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality, safety, and clinical outcomes.
