Adaptive Home Health is seeking an RN Case Manager to deliver high-acuity home health care in the field across Texas. You will own assessments, manage visits, and coordinate care with physicians and families to improve patient outcomes.

The role emphasizes autonomy, collaboration with a support team, and a productivity-based environment with a focus on wound care and infusions. Full-time, some weekend coverage as needed.

Qualifications

• Current Texas RN license.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR/BLS required.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with focus on wound care and infusions.
• Build and maintain plan of care through communication with physicians and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Coordinate services and follow-ups to meet goals.
