Job summary

Adaptive Home Health is seeking an RN Case Manager to deliver skilled home health care in the field and advocate for patients and families across Texas.

You will own assessments and OASIS, provide wound care and infusions, and coordinate with physicians and care teams to keep plans of care aligned and patient progress on track. This role offers autonomy and impact in a mission-driven environment.

Qualifications

• Texas RN license is required.
• 3+ years of clinical nursing experience.
• Experience with wound care and infusions.
• CPR (BLS) certification.
• Texas driver's license and willingness to work in the field.
• Strong time management and organization in a high-autonomy environment.
• Clear, professional communication with patients, families, and providers.

Responsibilities

• Own patient assessments and OASIS documentation.
• Deliver high-acuity home health care, focusing on wound care and infusion therapy.
• Build and maintain a clear plan of care through consistent communication with physicians, care team members, patients, and families.
• Serve as the patients advocate and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination, timely follow-ups, and barrier resolution to keep patients progressing toward goals.
• Coordinate with internal clinical and operational support teams to ensure plan of care is executed.
• Administer medications and treatments as ordered, and provide teaching to patients and caregivers.
• Coordinate supervision of LVNs and HHAs where applicable.
• Participate in case conferences and in-services to align on quality, safety, and clinical outcomes.
