Job summary

Adaptive Home Health is seeking a Registered Nurse Case Manager to deliver high-acuity home health care in the field across Texas. You will own patient assessments, manage care plans, and coordinate services to ensure timely, aligned outcomes with physicians and families.

You will advocate for patients, supervise LVNs and HHAs, and participate in case conferences to drive quality and safety while using our charting and scheduling products to support efficient visits.

Qualifications

• Texas RN license required.
• 3+ years of clinical nursing experience.
• Wound care and infusions experience.
• CPR (BLS) required.
• Texas driver’s license and field-based work.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care focusing on wound care and infusion therapy.
• Build and maintain a plan of care through communication with physicians, care team, patients, and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination and timely follow-ups.
• Coordinate with internal teams to ensure care is delivered safely.
• Provide supervision to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality and outcomes.
