Job summary

Adaptive Home Health, LLC is seeking a Physical Therapist to manage an in-home caseload across a field-based territory in Texas. The role focuses on comprehensive PT visits, gait and balance training, transfers, and functional mobility with an emphasis on home safety and fall prevention.

You will coordinate with RNs and the clinical team, escalate changes promptly, and document thoroughly using HCHB. A strong driving focus and ability to travel daily are required for this field-based position.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Clear escalation of changes in condition to RN/MD with rationale.
• Excellent communication and documentation skills.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and willingness to travel across territory.
• Ability to carry/transport basic therapy equipment.
• Competency in core home health PT scope: gait/balance, transfers, strengthening, mobility, home safety.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set measurable goals and track progress; adjust treatment as needed.
• Drive multidisciplinary coordination with the care team.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in HCHB.
