Job summary

Adaptive Home Health, LLC in Texas is seeking a Physical Therapist to lead in-home care on a field-based, full-time basis. You will manage caseloads, perform assessments, design plans of care, and coordinate with RN/MD while traveling across the territory.

You will use Homecare Homebase for documentation and will be supported by a productivity model. The compensation range is $100K-$130K per year with field-based schedules and occasional weekend coverage as needed.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication and thorough documentation.
• Reliable follow-through on orders, frequencies and goals.
• Current driver’s license with daily travel across assigned territory.
• Comfort carrying/transporting basic therapy equipment.

Responsibilities

• Own the caseload as a case manager and oversee therapy plans, progression and outcomes.
• Perform comprehensive in-home assessments and deliver skilled PT visits.
• Develop and execute plans of care: gait/balance, transfers, strengthening, mobility, home safety.
• Set measurable goals, track progress, and adjust treatment as needed.
• Coordinate with RN/MD and care team to align orders and care delivery.
• Supervise PTA where appropriate and document activities in the charting system.
