Job summary

Adaptive Home Health is seeking a Field Based Physical Therapist (PT) to manage a full caseload delivering in-home skilled PT visits across Texas. You will assess, treat, and progress patients toward functional mobility goals, with a focus on gait, balance, transfers, and home safety.

You will collaborate with a multidisciplinary team, document thoroughly in HCHB, and participate in scheduling and routing improvements as the company rolls out new tech tools.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and comfort working independently in the field.
• Excellent communication and thorough documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and willingness to travel daily.
• Ability to carry/transport basic therapy equipment.
• Competency in core home health PT scope.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, and outcomes.
• Complete in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care for gait/balance, transfers, strengthening, mobility, and home safety.
• Set measurable goals, track progress, and adjust treatment as needed.
• Coordinate with the care team and communicate condition changes.
• Supervise PTAs where appropriate and document care.
• Coach patients/caregivers with practical home programs and home exercises.
• Document promptly in HCHB with clear, defensible notes.
