Job summary

Adaptive Home Health is seeking a Physical Therapist in Detroit to deliver excellent home health care while building our clinical standard and patient trust. You will own a full caseload, perform in-home assessments, and guide a plan of care focused on gait, transfers, and functional mobility.

You will coordinate with the care team, supervise PTAs as needed, and coach patients and families. A Michigan license and daily travel are required, with a full-time schedule.

Qualifications

  • Active Michigan PT license in good standing.
  • Strong clinical judgment; comfortable working independently in the field.
  • Able to escalate changes to RN/MD with clear rationale.
  • Strong communication and documentation.
  • Reliable follow-through on orders and patient goals.
  • Current driver’s license; comfortable with daily travel.
  • Able to carry/transport basic therapy equipment.
  • Competent in core home health PT scope: gait/balance, transfers, strengthening.
  • Patient/caregiver coaching with practical home programs.

Responsibilities

Job description

  • Role: Physical Therapist (PT)

  • Schedule: Full-time Monday–Friday + occasional weekends (as needed)

  • Location: Field Based

  • Compensation (Productivity Points): $75.00 / point | $100,000-$130,000/year

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives
  1. Carry a full, high-quality caseload: ramp to ~28 productivity points/week at target visit quality.

  2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

  3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

  4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own
  • Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

  • Complete comprehensive in-home assessments and deliver skilled PT visits.

  • Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

  • Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.

  • Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

  • Manage treatment and outcomes directly, including PTA supervision where appropriate.

  • Coach patients and caregivers with practical home programs that actually get done.

  • Document promptly and thoroughly — defensible, efficient, and consistent.

Must-haves

    • Patient/caregiver coaching with practical home programs

  • Prior home health experience

  • Deeper clinical experience in high-signal PT populations

  • Experience supervising/mentoring PTAs and collaborating on care plans
