Job summary

Adaptive Home Health is seeking a Physical Therapist for a field-based role in Detroit. You will manage a full caseload with a target of ~28 productivity points per week and deliver in-home PT visits, gait/balance training, and safety assessments.

Must hold an active Michigan PT license and be comfortable traveling across the territory. Competitive compensation ranges from $100,000 to $130,000 per year with full-time benefits and sign-on opportunities.

Qualifications

• Ability to escalate changes in condition to RN/MD with clear rationale.

Responsibilities

• Own the caseload as the case manager: therapy plan, progression, outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set measurable goals, track progress, and adjust treatment quickly when needed.
• Drive multidisciplinary coordination with care team.
• Manage treatment and outcomes directly, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

• Carry a full, high-quality caseload: ramp to ~28 productivity points/week at target visit quality.
• Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
• Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
• Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own

• Document promptly and thoroughly in Homecare Homebase (HCHB) - defensible, efficient, and consistent.

Must-haves

• Active Michigan PT license (in good standing)
• Strong clinical judgment and comfort working independently in the field
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
• Strong communication and documentation
• Reliable follow-through on orders, frequencies, and patient goals
• Current driver’s license and comfort with daily travel across assigned territory
• Able to carry/transport basic therapy equipment as needed
• Competency in core home health PT scope
• Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
• Strengthening, functional mobility, and home safety assessment/modification education
• Patient/caregiver coaching with practical home programs

Nice-to-haves

• Prior home health experience
• Homecare Homebase (HCHB) experience (or similar home health EMR)
• Deeper clinical experience in high-signal PT populations
• Experience supervising/mentoring PTAs and collaborating on care plans
• Strong organization and time management

Our guarantee

• Full-time target: ~28 productivity points/week; clinicians earn more for exceeding it.
• Guaranteed floor: ~24 points/week for the first 90 days - if your caseload hasn't filled, you're still paid the floor.
