Job summary

You will work with a multidisciplinary team to help members progress toward outcomes and ensure high-quality, cost-effective care. Responsibilities include completing assessments, developing care plans with members and providers, conducting visits, monitoring progress, and promoting integration of services.

Qualifications

• Minimum 2 years healthcare experience, preferably in behavioral health.
• Active, unrestricted license in state of practice.
• Valid driver’s license, reliable transportation.
• Experience with severe mental health concerns and substance use disorders.
• Knowledge of whole person care, chronic conditions, and discharge planning.
• Data entry skills and experience using a clinical platform.
• Excellent verbal and written communication skills.
• Proficiency with Microsoft Office.

Responsibilities

• Completes comprehensive behavioral health assessments per timelines and identifies who may qualify for care coordination/case management.
• Develops and implements care plans with member, caregiver, physician and other healthcare professionals and support network.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plans, documents interventions and goal achievement, and suggests changes.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and informal ICT collaboration.
• Uses motivational interviewing to educate, support and motivate change.
• Assesses barriers to care and provides coordination to address concerns.
• May provide consultation to peers as needed.
• Travel 25-40% may be required.
