You will conduct comprehensive assessments, develop care plans with members and care teams, perform home visits as needed, monitor progress, and promote integration of behavioral health with LTSS and community resources. Travel 25-40% may be required.

Qualificações

• 2+ years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician with active license in state of practice.
• Valid driver’s license and reliable transportation for travel requirements.
• Experience with severe mental health conditions, substance use, and foster care.
• Knowledge of whole person care, chronic conditions, and discharge planning coordination.
• Data entry skills and experience with a clinical platform.
• Excellent verbal and written communication skills.
• Microsoft Office suite proficiency.

Responsabilidades

• Completes comprehensive behavioral health assessments and determines care coordination eligibility.
• Develops and implements care plans with member, caregiver, physician, and team.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload and outreach.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration with ICT.
• Uses motivational interviewing and Molina guidelines to support change.
• Assesses barriers to care and provides coordination assistance.
• May provide consultation and recommendations to peers.
• 25-40% travel may be required.
